New Clients:   

• Please fill out the Health Intake Form asap. Failure to fill out the form within 72 hours before your appointment may result in cancellation.
• All appointments are required to be confirmed with a credit card on file.
• A credit card is required when booking to confirm your appointment.
• Spa Liz and Co. requires 24-hour notice for appointment cancellation without a fee.
• Cancel within 24 hours of your appointment. It will result in 50% of the total service fees being charged to the credit card on file.
• No show/ no call will result in 100% of the total service fees being charged to the credit card on file.
• One service booked will be deducted from the package if using a package.
• If no credit card is on file, you will receive an online payable invoice that must be paid before another appointment can be booked.
